Position Summary
The Certified Wound Ostomy Nurse (CWON) is accountable for direct and indirect quality care for patients according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. The CWON will function as an educator and consultant to the Home Health Services staff, patients, and in the community as needed. The CWON will coordinate patient care activities with other members of the health care team in a collegial relationship.
Position Specific Responsibilities
• Utilizing the nursing process, assesses patient status, intervenes appropriately, and evaluates patient response.
• Establishes a formal written plan of care for referred patients, documents specific nursing orders, and develops goals specifying expected outcomes of intervention.
• Assesses health knowledge, learning abilities and educational needs of the wound care and ostomy patients and families and develops appropriate plans for teaching. Utilizes all available resources and consults as necessary with members of the health care team in planning and developing individualized teaching and discharge plans.
• Completes all necessary documentation according to regulatory standards.
• Functions as a knowledgeable resource person to the nursing staff in areas of expertise, related to ostomy care, prevention and treatment of wounds of all types, and management of the incontinent patient. Serves as a resource person to clinics, physician's offices, and businesses in the areas of expertise. Reviews literature, methodology, outcomes, and implications of nursing research, and disseminates this information to clinical and medical staff.
• Identifies staff learning needs, provides the necessary staff education in collaboration with RN Educator. Participates as preceptor in the orientation of new staff and/or students as assigned by the supervisor.
• Works closely with Home Health Services supply management, medical equipment vendors, and outside vendors as needed to coordinate availability of needed wound care products, and to improve quality of service to patients. Participates in product evaluations of new equipment and supplies related to areas of expertise.
• Actively participates in Home Health Services quality management programs and department meetings.
• Continues professional development and demonstrates advancement of professional nursing practice through both internal and external continuing education opportunities, membership in professional organizations, participation on Home Health Services committees.

Pay Range: $53.00 - $78.00
Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
