Job Summary:

The Transplant Social Worker provides comprehensive social work services to patients and families through all phases of transplant, from the date of initial referral through transplant hospitalization and during the posttransplant period. As a member of the transplant team, the social worker provides patient education and emotional support, is responsible for referrals to community agencies, coordinates patient care with other team members, and assists with concrete services.

Core Responsibilities and Essential Functions:

Psychosocial Assessment: Conducts a psychosocial assessment to determine the patient's psychological and social readiness for the demands and stresses associated with transplantation, recovery, and rehabilitation. Explores the overall quality of life, support systems, behavioral, psychological, and risk factors present that may impact overall success with transplantation. The assessment is utilized to determine transplant candidacy in conjunction with the multidisciplinary team. Consultation/Communication: Collaborate with the patients, families, caregivers, legal guardians, and healthcare team of factors that may potentially inhibit their understanding of the patient's pre/posthospital care. Provide concrete services such as assistance with housing, transportation, benefits expansion, and referrals to community resources, i.e. fundraising foundations. Assists with the return to work. Maintain ongoing communication with the multidisciplinary team to enhance care planning. Documentation: Provides communication and substantiation of services rendered through clear, concise, and timely documentation in the patient's electronic medical records. Self-Management: Effectively prioritizes referrals. Produces acceptable work volume and effectively meets established deadlines. Works independently; able to act in an autonomous self-directed manner. Displays good judgment and problem-solving skills. Education: Provides education to physicians, staff, patients, and families regarding coordination of care and related issues. Familiar with transplant regulatory policies. Maintains job-related knowledgeable skills, pursues self-development opportunities. Facilitates and maintains an environment that is conducive to the clinical education of social work students - when needed. Maintains BLS certification and yearly Child Abuse Training certification

Masters Social Work

All certifications are required upon hire unless otherwise stated.

BLS-I - Basic Life Support - Instructor or BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life SupportLMSW - Lic Master Social Worker GA Upon Hire Required

Required Minimum Experience:

Minimum 2 years Healthcare or hospital setting coordinating post-discharge services. Required

Excellent clinical assessment skills Strong organizational skills Ability to work independently and team setting
