YOUR CONTRIBUTION TO OUR SUCCESS!A Patient Care Manager contributes to AHF by utilizing experience, skills, education, and a thorough understanding of the Ryan White Care Act Program within their specific area. With a caring and knowledgeable focus, you will work with our providers and Healthcare Center team so that every Ryan White patient receives the support they require and access to available resources. This will include providing education and assistance so that eligibility for the program is understood. Working directly within the Healthcare Center, you will meet with patients and complete health risk/needs assessments, develop integrated care plans, and provide ongoing education to patients so that they understand their diagnoses and medications. You may also conduct home visits as clinically appropriate to complete assessments or follow up on patient needs, ensuring equitable access for patients with transportation or mobility barriers. For Ryan White contracts that combine psychosocial needs within Medical Care Coordination, you will understand mental health needs, addiction challenges, and basic needs such as housing, food, and clothing and develop relationships with programs and agencies serving those needs and facilitate referrals, support, and assistance for your patients who lack those services. You will also maintain an active and updated network of community resources to support housing stability, food access, transportation, behavioral health, legal aid, and other social determinants of health. You will also conduct ongoing retention calls to ensure no gap in care and uphold radical cooperation across divisions. As part of your role in retention and re-engagement, you will help identify and support clients at risk for falling out of care, coordinate with the Retention Outreach Specialist and other MCC teams, and implement interventions that reflect each client's stage of change and acuity level.
A Patient Care Manager enhances the reputation of AHF by utilizing your case management experience. You will meet our patients where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting. With your skills of collaboration and engagement, you will bring the essence of the Ryan White Care Act - the safety net for uninsured HIV/AIDS patients - to our most vulnerable population. By establishing Health Risk/Needs Assessments, Integrated Care Plans, referrals, behavioral health interventions, and support, your work will directly contribute to AHF’s success in meeting the deliverables of our Ryan White contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services. You will assure the coordination, follow-up, and completion of medical treatments and/or care plan interventions so that these services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care through ongoing assessment of the client’s and other key family members’ needs and personal support systems. You will verify the client’s eligibility for the ACA (Affordable Care Act) program and initiate enrollment for those who qualify to ensure broader access to healthcare services.

The Patient Care Manager will work as a part of a close-knit team to develop relationships with community programs and agencies serving patient needs and facilitate referrals, support, and assistance for patients who need those services. Your interventions and actions will support the patient’s ability to understand better their health status, their social determinants of health, and how they can work to achieve their health and social needs goals. You will provide brief interventions focused on a long-term achievable goal, and the client and the PCM will work together on each step towards viral suppression, better health outcomes, and improved quality of life. Interventions may include education on risk reduction counseling, behavioral health support, and disclosure assistance. These interventions are guided by evidence-based models like motivational interviewing and are tracked in accordance with AHF documentation standards. You will also provide addiction education and early intervention services as part of your role. This includes supporting patients who are actively using substances, those who are in recovery, and those expressing readiness for change. Your responsibilities may include brief counseling on substance use, identifying patterns or triggers, and linking patients to appropriate treatment resources. In higher acuity cases, you will coordinate referrals to specialized addiction treatment programs and continue follow-up support to encourage engagement and retention in care. You and the MCC team will meet directly with patients’ primary care providers to conduct case conferences, which will enhance the coordination of care. You will lead discussions involving barriers to care, such as mental health and addiction challenges. You will collaborate with the RNMCM and the providers to mutually develop and establish an integrated care plan with each client that identifies patient needs, goals, and a planned course of action to promote readiness for and adherence to complex HIV/AIDS treatments (i.e., counseling, special programs), and meets identified needs; monitor and document the changing needs of the client through personal contact and by reviewing the care plan at each monthly follow-up. To ensure the quality of services, you will also conduct monthly peer reviews to assess documentation, care planning, and service delivery, helping ensure that AHF standards for the MCC program are consistently met. Your contributions directly support the health and well-being of people living with HIV by providing timely, compassionate care through accurate documentation, effective collaboration, and meaningful patient engagement, helping MCC meet program goals while centering the needs of those we serve.
Licenses and Certifications:
Master’s degree in one of these disciplines: Social Work, Counseling, Psychology, Marriage and Family Counseling, and/or Human Services.
A minimum of 3 years of Case Management experience.
Knowledge of Ryan White Services is preferable.
Competence in Cultural and Linguistic Diversity.
