Schedule Details/Additional Information:

This role is based within an inpatient hospital setting. The standard work schedule is Monday to Friday, 8:00 a.m. to 4:30 p.m.

Pay Range

$28.55 - $42.85

Major ResponsibilitiesResponds to palliative care referrals within 24-72 hours based on established referral criteria.Assesses, plans, and implements appropriate interventions related to the psychosocial needs of Palliative Care patients and their families. Utilizes training to address end-of-life issues throughout the hospital.Documents assessments and other relevant data related to interventions in the patients’ medical records. Provides individualized services to patients and families throughout hospitalization. Services may include but are not limited to completion of psychosocial assessment and re-assessment as needed, supportive counseling, crisis intervention, emotional support, bereavement counseling, providing education on new diagnosis, and ensuring understanding of overall health.Stays informed about community resources and assists patients and families in utilizing appropriate resources that offer support during and after hospitalization.Acts as a liaison between family members, physicians, and hospital staff in complex cases involving multiple physicians. Manages critical care cases in the ICU.Educates hospital staff and physicians about Palliative Care and promotes the program to prospective customers such as staff nurses and physicians.Serves as an integral member of the multidisciplinary care team and regularly attends Palliative Care meetings.Provides support and supervision to interns, medical students, and Critical Care Academy nurses, including orientation and training for new staff.

Minimum Job Requirements:Master’s degree in family therapy, counseling social work or related disciplineMinimum two (2) years’ experience in field of social work, counseling or family therapy or completion of 1 year internship in Palliative Care.Prefer medical experience or working with families with life limiting or life-threatening illnesses.
