The Care Coordination Services team is looking for a Sr Project Analyst who can effectively manage complex projects, maintain high standards of operational excellence, and serve as a key point of contact for clients.
The ideal candidate will have a proven track record of successfully managing program implementations and the ability to quickly learn the systems and processes involved in Care Coordination Pathways.
**Job Duties and Responsibilities:**
1. Overall program and project management of CCP for All
2. Build the pipeline: Work with CS and Sales Ops to communicate the concept with clients, get them to sign up, and manage the waves of implementation (forecasting volume)
3. Lead new CCP for All Implementation projects including:
+ a. 40 new clients to implement in remainder of 2025 (7 months)
+ b. Communicates with the Client Success team and Client to ensure clarity of roles, responsibilities, and expectations.
+ c. Build project plan for each new client and drive project progress
- Client contracting approval
- Client scripting approval
- Gather client specific pathway information and design decisions
- Work with Engineering teams for technical pieces
- Work with Salesforce engineer to add client specific data (member services number/find a pcp url
- Work with each client and reporting team to set up and deliver standard reporting
- Communicate with care coordination agents updates on live implementations
- Determine and communicate to managers when volume dictates additional agent resources are needed
4. Identifies and works to resolve issues that would delay the timeline of each project implementation
5. Ongoing Care Coordination Operations
+ Complete data audits on new clients to ensure Signify and Client reporting are accurate
+ Work with Client Success team to answer questions, fulfill ad hoc requests and reports
+ Update Care Coordination Reporting with weekly/monthly/quarterly metrics
6. This cycle will repeat as we add more Pathways to “CCP for All”
7. Assist with other team projects with multi-client rollout plans (e.g. Pre-Visit Questionnaire, Referrals)
**Basic Qualifications:**
+ 2+ years of project management experience, preferably within a Healthcare Organization, Operations, or Technology function, with the ability to manage multiple projects simultaneously.
+ Bachelor’s degree in Business, Supply Chain Management, Operations Management, Finance, or related field
+ Excellent problem solving abilities and a proactive approach to addressing operational challenges
+ Reporting and analytics experience with the ability to create detailed reports and dashboards
+ Knowledge of Salesforce is a plus
**Preferred Qualifications:**
+ Experience with project management and workflow tools like SmartSheets, Jira, Asana, or Lucid Charts
+ Project Management Professional (PMP) certification is a plus
+ Experience managing change and maintaining excellent relations with executives, customers, Operations and Product Management staff at all levels

**Pay Range**
The typical pay range for this role is:
$46,988.00 - $102,000.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
