As a crucial member of our home‑based care team, you will:

Champion Elara Caring’s mission and philosophyAssess and evaluate patient needs, establishing a personalized Plan of CarePerform initial OASIS assessments and ongoing re-evaluationsEnsure patient safety through thoughtful care planning and resource coordinationCollaborate with physicians, supervisors, and interdisciplinary team membersImplement and revise Plans of Care based on patient progressProvide direct speech-language pathology services according to clinical standardsSelect therapeutic materials tailored to patient needsEducate and support patients and caregivers on home therapy techniquesMaintain accurate, timely documentation and communicationWork in a dynamic, physically demanding environment with flexibility for irregular hours✅ What You’ll NeedMaster’s degree from an accredited program in Speech-Language PathologyCurrent, unrestricted SLP license in IndianaMinimum 1 year of clinical SLP experienceAbility to travel 50% within your service areaReliable vehicle, valid driver’s license, and auto insuranceAbility to lift 50–100 lbs. and assist with patient transfers📍 Location
