Specialty Pharmacist - Ambulatory
Location: Rockford, ILHours: 40 hours per week

Performs all functions of pharmaceutical care required of an ambulatory pharmacist. Responsible for providing medication therapy management services to assure that drug therapy is safe, effective, efficient and cost effective. Assists patients, providers and partners with chronic disease state management and education as part of an interdisciplinary team. Completes patient assessments and follow-up necessary to comply with specialty pharmacy accreditation standards. Collects and analyzes data for process improvement projects. Responsible for assisting or leading projects involving specialty pharmacy which may include sharing information and improving processes with retail pharmacy partners. Will be required to take after hours call on a rotation basis. Performs other duties as assigned or as necessity dictates.
EDUCATION AND EXPERIENCE
Minimum - Graduate of an accredited Pharmacy program
Preferred - Completion of an ASHP Accredited Residency training program
CERTIFICATION/LICENSURE
Pharmacist license (or eligible for licensure) in the State of Illinois and/or Wisconsin based on practice setting
Pharmacist license in both Wisconsin and Illinois may be required
Approved CPR or BLS certification required within 90 days
ESSENTIAL DUTIES AND RESPONSIBILITIES
Maintains professionalism and conducts self as a role model, serving as a resource for the other retail and clinic partners
Partner with providers and Mercyhealth team members to promote the use of entity owned pharmacies
Collaborate with leadership to ensure strategic goals for the department are achieved
Serve as the clinical lead for the specialty pharmacy program for Mercyhealth clinics and retail pharmacy in Rockford
Is an expert outpatient pharmacist who relies on skills of comprehensive knowledge and experience
Monitor patients’ response to medication therapy and consults with providers to make appropriate adjustments
Provides supervision for pharmacy technicians and/or prior authorization coordinators to ensure efficient quality care for Mercyhealth patients
Supports prior authorization process by submitting, appealing or coordinating with patient’s insurance plans
May float to provide coverage of pharmacy needs within the outpatient setting
Assist with academic functions such as serving as a preceptor for pharmacy students
Maintains practice by ongoing professional literature review, continuing education and self- evaluation
Maintains competencies in federal and state regulations related to pharmacy practice
Provide patient consultations in person or via telehealth to optimize medication therapy for patients with complex or chronic conditions to improve patient outcomes and ensure safe, effective, and appropriate medication use.
