The Social Services Department in Manhattan Division of The New Jewish Home is seeking for a full time temporary Social Worker to join their team! The duration of work is between 4 to 5 months and maybe extended if needed.  Job Responsibilities: Completing MDS and biopsychosocial assessments. Facilitate care plan meetings & discharge planning. Implement Advanced Directives and End of Life Planning. Completing all required documentation in the medical record ( EMR) Coordination of care and services including ongoing monitoring of an appropriate and effective person-centered care plan; Strong Clinical, Communication and Organizational Skills
