We are currently hiring a Full Time Social Worker to join our Social Services Team!

 

Responsibilities:

This position is responsible for comprehensive assessment and coordination of services required to meet the individual’s psychosocial/mood state, behavioral and discharge needs of each resident. In addition, this position facilitates the interdisciplinary teams’ coordination of resident needs, as well as the development and implementation of a safe and effective discharge plan.  Obtains a strong skill set on discharge planning and coordination of external services for patients.

Applicants with rehabilitation, sub-acute experience are encouraged to apply and will be given priority consideration.

Qualifications:

Bachelor Degree in Social Work or related field required.Possess a minimum of 2 years’ experience in a health care setting, preferable in a rehab departmentDemonstrated ability to execute discharge planning efficiently in accordance with facility requirements.Strong problem solving and organizational skills.Sound knowledge of community resources, entitlements, and insurers.Strong crisis intervention skills required.Excellent verbal and written interpersonal skills that promote family engagement, as well as collaboration across interdisciplinary teams.
