Job Title: Care Coordination Social Worker IIJob Description
The Care Coordination Social Worker II supports patients and families by completing comprehensive assessments, providing crisis intervention, coordinating care, and connecting members with community resources. This role works closely with healthcare providers, community agencies, and internal teams to ensure members receive the support they need. Accurate documentation, case presentations, and strong advocacy are essential parts of the position.
This is a hybrid role with limited onsite time in the Santa Barbara office. Local candidates (Ventura, SB, SLO counties) are preferred, but candidates anywhere in CA will be considered.

This is a Contract position based out of Santa Barbara, CA.
Pay and BenefitsThe pay range for this position is $39.50 - $39.50/hr.
