Position Summary
Under general supervision and according to established practices and professional standards, assists patients and their families to resolve personal and environmental problems which prohibit or limit patient from obtaining maximal benefits from Behavioral Health Services. Evaluates psychosocial functioning of patients and families and counsels them as necessary. Exchanges information with other Hospital professionals as well as representatives of outside agencies. Assists with patient discharge planning. Conducts psycho-educational groups. Assists in utilization review.
Job Specifics
Conducts psycho-educational groups for patients. Develops and implements a plan to assist the patient and family in resolving identified problems. Provides counseling and support to patients and their families in order to assist in the solution of psychosocial, behavioral health related problems such as suicidal ideation, depression, psychosis, and mania. Assists in the utilization management of patients in the Behavioral Health Services Unit. Communicates effectively and collaboratively with all members of the healthcare team: Reviews patient/family care needs with nursing personnel and physicians. Provides advice concerning patient’s social and emotional needs and solicits input regarding patient assessment. Establishes and maintains a cooperative working relationship with representatives of various community social agencies, extended care facilities, and other resources essential to effective post-discharge care. Provides program/information on social work services to outside organizations when requested. Records information concerning social assessment, patient counseling, discharge planning, and other appropriate information on the patient’s chart. Performs a formal psychosocial history when deemed necessary or when requested. Maintains appropriated records for the reporting of individual and departmental statistics on a regular basis. Demonstrates commitment/support of departmental goals, policies and procedures to ensure efficient and effective operations. Performs related professional services such as program development and visitation of community patient care facilities. Remains abreast of advances in the field of social work by reviewing pertinent literature, in-service training, seminars, and discussion with peers. Remains abreast of community resources, Federal and State social programs, insurance programs, etc. Completes job responsibilities within required time frame, according to established schedules or work flow requirements. Assume responsibility for attending staff meetings, mandatory education as designated by department and hospital policy. Demonstrates support for all hospital and system initiatives and resulting operational changes.
Education/Experience
Level of knowledge equivalent to that ordinarily acquired through completion of a Baccalaureate Degree in Social Work. Master Degree in social work preferred.One to two (2) years of experience in a healthcare setting in order to:
acquire diagnostic interviewing skills; acquire counseling skills and knowledge of techniques; become familiar with local community agencies and facilities; learn to develop adequate treatment plans; and; acquire suitable interpersonal and analytical skills.
Certified Social Worker in the State of Indiana or certification eligible.
