**Job Summary and Responsibilities** **Position Summary:** Under direct supervision, the incumbent identifies and evaluates potential clients as total persons and identifies the functional limitations which impede independent living. This position reports to the RN Supervisor. **Job Requirements** **Minimum Qualifications:** + Bachelors Other in social work from an accredited school + Two years of experience providing social service in one or more of the fields of aging health and long term care services + Basic Life Support - CPR (Must be through the American Heart Association) **Where You'll Work** Dignity Health Connected Living has been serving seniors in Shasta County since 1968.
