The pay for this role is $40 - $47 per DOE

We have a fantastic opportunity for an Assisted Living Wellness Nurse to join our team! This role is a vital part in the care of our residents and and to be a valued member of our community! We look forward to learning more about how, together, we can Transform the Perception of Age!

Our Licensed Practical Nurses:

Provides direct nursing care to residents in accordance with physician orders and state regulations

Administers medications and treatments, monitors effectiveness and reports changes in condition

Completes and maintains accurate clinical documentation, MARs, TARs, and resident records

Conducts resident assessments and participates in service plan development and reviews

Coordinates with physicians, pharmacies, and external providers as needed

Communicates resident condition changes to physicians, families, and leadership

Supervises and supports CNAs/med techs; provides guidance and oversight of care delivery

Ensures compliance with infection control practices, medication management, and regulatory standard

Participates in admissions, discharges, incident reporting, and resident care meetings

Responds to resident and family concerns and assists with resolution

What you need to succeed!

Be a graduate of an accredited LPN Program

Have an active LPN license in the state, in good standing

Experience in a Nursing Home (Assisted Living or Long-Term Care preferred, but not required)

Have flexibility, able to work the occasional weekend/holiday as needed.

Have knowledge of DHS Regulations

Possess excellent interpersonal and communication skills and the ability to work well and collaboratively with team members and external individuals. 

Completion of a background check, drug screen. Other pre-employment screenings may include physical, TB test, and fingerprinting per state/local guidelines.
