Job Description
A client of Insight Global is seeking a Home Health Registered Nurse Senior Case Manager to oversee and coordinate patient care in a home‑based setting. This role requires strong clinical judgment, experience in case management, and the ability to work closely with multiple disciplines. The Senior Case Manager will help develop individualized care plans, support field staff, ensure regulatory compliance, and provide direct patient care when needed.
Top Responsibilities
1. Lead, Train, and Support Field Nursing Staff
Provide ongoing guidance, mentorship, and accountability for RN leaders, field nurses, and home health aides.
Oversee daily care delivery, ensuring staff follow evidence‑based practices, care plans, and quality standards.
Conduct annual reviews for direct reports and manage PTO requests.
2. Coordinate and Manage Patient Care
Serve as the primary case manager for assigned patients, coordinating all care services, community resources, and medical appointments.
Facilitate communication between all parties involved in care—including physicians, therapists, and caregivers—to ensure smooth continuity.
Ensure adherence to state, federal, and agency policies.
Complete supervisory visits for LPNs (every 60 days) and HHAs (every 14 days).
Lead the full case management and care‑plan development process for new admissions.
3. Ensure Quality, Safety, and Regulatory Compliance
Maintain accurate, timely documentation in alignment with regulations and internal policies.
Follow professional standards of practice, including providing direct patient care when required.
Monitor and coordinate care across all disciplines within assigned caseloads.
Participate in quality improvement efforts, evaluating clinical data to identify trends and areas for improvement.
4. Direct Patient Care & Timely Documentation
Conduct initial and ongoing clinical assessments to evaluate patient needs and progress.
Create and update individualized care plans in collaboration with the interdisciplinary team.
Investigate and resolve patient or family concerns as assigned by leadership.
Participate in the on‑call rotation.
Ensure all case‑management tasks are completed for patients in the assigned geographic area.
5. Support Admissions & Cross‑Department Collaboration
Coordinate with patient representatives, advocacy groups, and other stakeholders involved in the case.
Support new admissions and ensure smooth onboarding into agency services.
Participate in weekly meetings with clinical leadership.
Provide patient and family education related to conditions, medications, treatments, and preventive care.
Assist in crisis situations by coordinating resources, providing intervention strategies, and notifying necessary departments.
Compensation:
$80,000 to $110,000.
Exact compensation may vary based on several factors, including skills, experience, and education.

Skills and Requirements
· Education: Bachelor of Science in Nursing (BSN) preferred or Associate Degree in Nursing (ADN)
· Licensure: Must hold a valid and active Registered Nurse (RN) license in the State of practice
· Experience: Minimum of 2-3 years of nursing experience required; at least 3 years of experience in home health, case management, or a related field strongly preferred.
· Certifications: Current Basic Life Support (BLS) certification
· Strong clinical assessment, critical thinking, and problem-solving skills
· Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare team members.
· Ability to adapt to a flexible work schedule and to be on-call if necessary.
· Proficiency in using electronic health records (EHRs) and medical documentation systems.
· Valid driver’s license and/or access to a reliable vehicle or transportation for State travel based on geographical location of clients
· Background check and drug screening required.
