The Care Transitions Clinical Specialist provides pivotal support and leadership within the Recuperative Care team by fostering a strengths-based, client-centered culture. This role coordinates medical services for clients, including assessment, education, and treatment of patient conditions. The position also oversees all medication management workflows, practices, policies, and patient self-administration of medications to minimize risk.
ResponsibilitiesSuccessfully complete the credentialing process if applicable.Collaborate as part of a multidisciplinary team supporting patients with homelessness and co-occurring disorders such as mental health, chronic medical conditions, and substance use disorders in a high acuity care facility with 24/7 patient support and monitoring.Train nursing, support staff, and new hires in nursing and patient support duties, medication management, and policies.Oversee nursing and support staff functions, ensuring compliance with established guidelines, protocols, and best practices.Perform RN duties for patients in medical respite, including preparing rooms, rooming patients, taking medical histories, recording vital signs, and performing venipuncture and immunizations as needed.Provide education and oversight to ensure best practices in client care and adherence to certifying and regulatory standards by all nursing and support staff.Essential SkillsCA RN License2+ years of supervisory experienceAdditional Skills & QualificationsProficiency in using EMR systems such as NexGenKnowledge of medical terminology and behavioral healthExperience as a patient coordinator or medical assistantWork EnvironmentThis role supervises up to 15 individuals. The work schedule is flexible, Monday through Friday from 11 am to 7 pm, with potential support for PM staff and one day starting around 6:00 am to support the night shift.
