Schedule: Monday through Friday, 8:30 AM to 4:30 PM, with flexibility as needed

Employment Type: Permanent

Compensation: $80,000 to $90,000

This role is ideal for a nurse who loves both clinical work and leadership. You will oversee care plans, support caregivers, advocate for program members, and ensure compliance across all medical and documentation practices. What You Will DoOversee healthcare services, interventions, and clinical needs for program membersParticipate in Individual Support Plans and case conferencesCoordinate medical services with primary care providers and external partnersAdvocate for the optimal health and well being of each program memberConduct preadmission reviews, assessments, and develop AFC plans of careEnsure regulatory compliance and maintain accurate, confidential medical filesRecruit, train, supervise, and support AFC caregiversComplete quality assurance tasks, including audits and policy reviewsCollaborate with multiple agencies to ensure coordinated, high quality careMaintain all required certifications and safety protocolsWhat You BringCurrent RN license in MassachusettsTwo or more years of recent hands on experience working with elderly or disabled adultsSupervisory experience preferredExperience working within AFC or DDS programs requiredEMR experience, ideally with IcentrixAbility to educate and train adult learnersStrong communication and organizational skillsBasic computer proficiency and comfort with remote communication (Zoom)Valid driver’s license, reliable personal vehicle, current insurance, and annual vehicle inspectionAbility to pass all background checks (CORI, SORI, fingerprinting)Preferred SkillsHome health nursing experienceStrong triage and medication management skillsExperience coordinating interdisciplinary care teamsWhy You Will Love Working HereA mission centered organization that makes a real difference in people’s livesA supportive culture that values teamwork and compassionWork life balance and a stable weekday scheduleFlexibility and potential to work from home once fully trainedA leadership role where your voice matters and your impact is visibleOpportunities to support caregivers, advocate for members, and strengthen community based careWork Environment

This is an onsite role with a Monday through Friday schedule. Although hours are consistent, some flexibility is needed to meet member or program needs. The team is experienced, collaborative, and committed to delivering thoughtful, person centered care.

Pay and Benefits

The pay range for this position is $80000.00 - $90000.00/yr.
