What You Will Do + Oversee healthcare services, interventions, and clinical needs for program members + Participate in Individual Support Plans and case conferences + Coordinate medical services with primary care providers and external partners + Advocate for the optimal health and well being of each program member + Conduct preadmission reviews, assessments, and develop AFC plans of care + Ensure regulatory compliance and maintain accurate, confidential medical files + Recruit, train, supervise, and support AFC caregivers + Complete quality assurance tasks, including audits and policy reviews + Collaborate with multiple agencies to ensure coordinated, high quality care + Maintain all required certifications and safety protocols What You Bring + Current RN license in Massachusetts + Two or more years of recent hands on experience working with elderly or disabled adults + Supervisory experience preferred + Experience working within AFC or DDS programs required + EMR experience, ideally with Icentrix + Ability to educate and train adult learners + Strong communication and organizational skills + Basic computer proficiency and comfort with remote communication (Zoom) + Valid driver’s license, reliable personal vehicle, current insurance, and annual vehicle inspection + Ability to pass all background checks (CORI, SORI, fingerprinting) Preferred Skills + Home health nursing experience + Strong triage and medication management skills + Experience coordinating interdisciplinary care teams Why You Will Love Working Here + A mission centered organization that makes a real difference in people’s lives + A supportive culture that values teamwork and compassion + Work life balance and a stable weekday schedule + Flexibility and potential to work from home once fully trained + A leadership role where your voice matters and your impact is visible + Opportunities to support caregivers, advocate for members, and strengthen community based care Work Environment This is an onsite role with a Monday through Friday schedule.
