Compensation 80,000 to 90,000 base salary depending on experience

About the Role

Join a mission driven team improving health outcomes for adults in community settings. As the RN Nurse Supervisor, you will lead clinical oversight across Adult Foster Care and developmental services programs, ensuring high quality care, regulatory compliance, and exceptional caregiver support. This role combines patient advocacy, care coordination, and people leadership with visible impact across the region.

What You Will Do

• Oversee and document members’ healthcare services, medical needs, and interventions while maintaining complete and confidential medical records.

• Participate in Individual Support Plans and case conferences and advocate for optimal member health in collaboration with Primary Care Physicians and partner agencies.

• Conduct preadmission reviews and comprehensive assessments and develop and update AFC plans of care aligned to regulatory requirements.

• Recruit, train, supervise, and support AFC caregivers; evaluate competencies and ensure adherence to policies, medication adherence, and safety protocols.

• Lead quality assurance activities including record audits, incident reviews, policy updates, and timely reporting of high profile cases.

• Coordinate labs, imaging, specialist referrals, and follow up care; triage needs and escalate concerns promptly.

• Utilize the EMR to document, track, and report clinical activity and outcomes; support efficient remote communication with members and caregivers.

• Travel within the region to conduct visits, assessments, training, and supervisory check ins.

What You Bring

• Current RN licensure in Massachusetts.

• Two or more years of recent experience providing direct care to elderly adults or adults with disabilities.

• One or more years of supervisory or lead experience preferred.

• Experience in AFC and or DDS programs required.

• EMR proficiency; Icentrix experience preferred.

• Ability to train adult learners and coach caregivers with clarity and empathy.

• Strong assessment, triage, and medication management oversight skills.

• Valid driver’s license, reliable personal vehicle, active auto insurance, and annual vehicle inspection.

• Ability to pass CORI, SORI, and National Fingerprint checks.

• Comfort using Zoom and basic productivity tools for remote communication and documentation.

Pay and Benefits

The pay range for this position is $80000.00 - $90000.00/yr.
