RN- Hospice / Home Care Coordinator, Full Time, Days, Summit NJ

Reports to: Manager of Business Development and Admissions

 

Duties & Responsibilities:

1. Performs Patient Assessments:

· Accurately assesses patient’s appropriateness for the Home Care or correct level of hospice care

· Acts as the face of the agency, meeting with patients and families to discuss the Hospice philosophy and benefit or Home Care regulations

· Confers with facility physicians, nurses, case managers and other providers to ensure the patient’s needs in the home are anticipated and appropriately addressed prior to discharge

· Ability to clearly and effectively communicate with Hospice medical director, staff, patients, caregivers, physicians and other departments within AHS

· Reviews appropriate documentation and confers with physician to obtain viable diagnosis for admission to service

· Participates in interdisciplinary discussion for appropriate level of care

· Completes referrals within Epic in a timely and accurate manner

· Communicates with CAC Manager and Assistant Managers on complex referrals

· Responsible for ordering required durable medical equipment for home hospice patients

· Knowledge of comfort care medications and ability to educate on same

Daily rounding with physicians and palliative team

Collaboration with physicians for appropriate discharge planning

· Maintains current knowledge of clinical issues through appropriate continuing education

· Maintains knowledge of regulatory and reimbursement issues, insurance contracts and negotiated rates

· Establishes self as resource for patients/families/physicians/discharge planning personnel/coworkers

· Confers with Referral Specialists on complex clinical referrals and provides oversite.

 

 

4. Demonstrates professional adaptability:

· Changes focus quickly and reprioritizes as necessary to provide continual flexibility

· Accepts new/additional assignments necessary to further agency goals.

 

5. Coding:

· Provides accurate and appropriately ordered ICD codes for patient diagnoses.

 

6. Performs other tasks as required.
