The RN Case Manager will perform telephonic case management for Medicare Advantage members in Florida. This role involves care coordination activities, developing care plans and assessments, conducting new referrals, contacting members, and building new cases using Athena EMR.
ResponsibilitiesPerform telephonic case management for Medicare Advantage members.Coordinate care activities and develop care plans and assessments.Conduct new referrals and contact members.Build and manage new cases in Athena (EMR).Essential SkillsValid RN license in Florida.Minimum of 2 years of case management experience in hospital/acute care, home health, or hospice.Proficiency with Athena EMR software/platform.Strong computer skills, including Microsoft Suites, Excel, and data entry.Additional Skills & QualificationsExperience handling a caseload of at least 73 patients.Background in patient care, triage, and utilization management.Prior experience in a fast-paced, high-acuity nursing environment.Work EnvironmentThis is a hybrid opportunity with work hours from Monday to Friday, 8am-5pm. The role requires a passion for case management work.
