The RN Case Manager plays a crucial role in the patient care delivery team within a level one trauma designation, ST-Elevation Myocardial Infarction (STEMI) receiving center, and Comprehensive Stroke and Chest Pain Center. This role requires the use of clinical expertise and independent judgment to perform the nursing process, including assessment, care planning, intervention, and evaluation, within a patient-centered care professional practice model.
ResponsibilitiesWork efficiently in a fast-paced environment, utilizing clinical expertise in case management functions.Maintain a solid working knowledge of case and utilization management methodologies.Ensure medical appropriateness criteria are met and documented from admission through discharge.Collaborate in a healthcare environment to implement an interdisciplinary plan of care with a comprehensive discharge plan.Reevaluate the plan of care based on the patient's changing needs and condition.Exhibit a positive mindset with changes in the workforce, clinical practice, and operational setting.Engage in opportunities to influence decision-making for bedside clinical practice.Work calmly and respond courteously under pressure.Essential SkillsMinimum of 2 years of acute inpatient experience.Experience in trauma level 1 hospital discharge planning.RN license with active and unencumbered California RN license.Active BLS certification from the American Heart Association.Bachelor’s degree in nursing.Minimum of two years inpatient acute care experience.Minimum of one year of hospital case management or hospital utilization management experience.Additional Skills & QualificationsNationally recognized certification in Utilization Management or Case Management preferred.Knowledge of state and federal healthcare regulations and accreditation.Work EnvironmentThis role offers the opportunity to work at a Magnet Recognized large tertiary academic medical center, which emphasizes continual learning and development. The case management team is extensive, providing substantial career-pathing opportunities. You will be working in a 400-bed, level one trauma acute setting. The position requires working every other weekend, and shifts are conducted within a collaborative and supportive healthcare environment.

Pay and BenefitsThe pay range for this position is $50.00 - $70.00/hr.
