Job Description
The RN Care Manager provides patient-centered care management services to patients and families within both the Emergency Center and inpatient settings. This role is crucial in identifying care coordination needs in collaboration with the inpatient and ambulatory care teams, as well as the patient's external support system.
Responsibilities
+ Identify care coordination needs in collaboration with inpatient and ambulatory care teams and patients' external support systems.
+ Participate in progressing patients towards discharge or the next level of care by identifying barriers and implementing appropriate solutions and interventions.
+ Work with patients and their external support systems to secure appropriate and safe transition plans in alignment with shared goals of care.
Essential Skills
+ Proficiency in acute care, case management, care management, and discharge planning.
+ Experience with EPIC and utilization management.
+ Exceptional organizational and time management skills.
+ Current Minnesota Registered Nurse (RN) license.
Additional Skills & Qualifications
+ Bachelor's Degree with at least one year of hospital-based med-surg nursing experience.
+ 1+ years of experience in case management, home care setup, and discharge planning within a hospital or nursing home setting.
Work Environment
The work environment includes a schedule of Monday through Friday, 8:00 AM to 4:30 PM. The role involves spending the majority of the day meeting with patients and their families to coordinate treatment, working closely with physicians to sign off on discharge. You will typically work with 12-15 patients per day. There is a two-week training period to learn specific systems.

This is a Contract position based out of Minneapolis, MN.
Pay and Benefits
The pay range for this position is $40.00 - $50.00/hr.
