Job Description

The RN Care Delivery Manager is responsible for supporting the nursing standards of excellence through advanced professional education to deliver high-quality care. This role involves serving as a member of the management team, collaborating with all departments and employees to oversee daily operations. The position requires providing guidance and implementing best practices to achieve the organization’s objectives, goals, and mission. The manager will cultivate an environment focused on improving experience, enhancing satisfaction, and ensuring retention. They are also responsible for providing care coordination that is sensitive to a wide range of diverse backgrounds and needs.

Responsibilities

Facilitate and complete the process of Interdisciplinary Team (IDT) meetings, Service Delivery Requests (SDR), and other meetings, including necessary attestations.Serve as a manager on call during non-operational hours and handle follow-ups and documentation.Balance and allocate staffing and resources to ensure adequate team assignments and seamless operations.Ensure documentation for participants is complete and accurate, covering enrollment, disenrollment, and medical records.Collaborate with the Transitions in Care Team to provide care management services across sites and communicate effectively with relevant team members.Ensure documentation complies with CLHC standards and federal/state regulations.Work with the Human Resources Department to conduct interviews, provide coaching, and handle disciplinary actions if needed.Assist in onboarding and professional development training of staff, ensuring compliance with policies and regulations.Evaluate the performance and competency of direct reports, providing feedback and conducting annual performance reviews.Act within the scope of the individual’s authority to practice.

Essential Skills

Current active and unrestricted RN license in New York State.Supervisory and management skills.Care management and case management expertise.Legally authorized to practice in the state.Medically cleared for communicable diseases with up-to-date immunizations.

Additional Skills & Qualifications

Minimum of two years of administrative experience in a management capacity in certified home health, long-term home health care, acute care, medical-surgical, critical care, nursing home, or diagnostic & treatment clinic.Customer service experience required.Managed long-term care insurance experience is beneficial.Experience working with a frail or elderly population, or must receive appropriate training from PACE organization upon hiring.Supervisory experience is required.

Work Environment

This position operates within an onsite medical clinic in a community healthcare setting. The work schedule is from 8:30 AM to 5:00 PM, totaling 37.5 hours per week. This is a Contract to Hire position based out of Flushing, NY.

Pay and Benefits

The pay range for this position is $60.00 - $65.00/hr.
