Description
Position Summary:
Responsible for tracking and follow up of clients in order to maintain an 80% follow up rate. Responsible for the administration of admissions, discharge and follow up assessments. Responsible for other program evaluation activities. Electronic Record Control- Enters knowledge assessments and other project data into the computer system. Assist Project Supervisors or Project Coordinator with quality assurance processes.
Essential Job Functions:
+ Develop and maintain basic statistical databases as needed
+ Collect quantitative and qualitative data utilizing the data collection worksheets.
+ Conducts admission, discharge, and follow-up evaluation assessments for clients.
+ Maintains system to identify timelines for individual client evaluations and ensures that evaluations and updates are conducted within designated timeframes.
+ Tracks clients to ensure availability for ongoing assessments, including developing outreach plan and incentive structures
+ Enter program-specific client data in the statistical databases
+ Enters data including:
+ GPRA data into SAIS system
+ Continuum of Care Web-based Homeless Reporting System
+ Other data collection tools as assigned.
+ Works with agency evaluation team to develop and implement comprehensive agency evaluation plan, including conducting client, referral-source, and employee and community evaluations utilizing focus groups and surveys.
+ With Evaluator, develop and implement system to monitor and report compliance status with funder's and program's required deliverables
+ Collect, analyze and report outcome data as required by all funders and programs
+ Participate in the development of agency/ program Performance Improvement Plans; collect and report on Performance Improvement status
+ Assist in roll out of Electronic Health Record and serve as onsite point person in implementation process.
+ Assist Program Supervisor in conducting documentation audits and other quality assurance activities
+ Other assignments requested by Supervisor.

Education:
Minimum High School Diploma, bachelor’s degree in a social service field, preferred. Must have at least one year experience collecting and entering data and one year experience in working with substance using and/or mental health clients. Should have excellent language and interpersonal skills and well organized in work habits. Strong verbal and written skills. Proficient in Microsoft Office software.

Physical Demands:
+ Requires mobility and physical activity. Having an adequate range of body motion and mobility, to work in the office environment including standing and walking (even and uneven surfaces) to interact with co-workers in the office environment, sitting for extended periods of time, bending, twisting, reaching, balancing, occasional lifting and carrying of up to 30 pounds. Constant use of a computer and telephone system is required, which includes coordination of eye and hand, and fine manipulation by the hands (typing, writing, and working with files). Requires the ability to operate a motor vehicle.

Talking is required to impart oral information to employees, clients, and the public and in those activities in which the employee is required to convey detailed or important spoken instructions to others accurately, loudly, or quickly.
+ Requires hearing: Hearing is required to receive and communicate detailed information through oral communication.
+ Requires seeing: Clarity of vision at 20 inches or less as well as vision adequate for driving a motor vehicle. This factor is required to complete paperwork for many of the employee’s essential job functions and when driving a motor vehicle.
+ The normal work routine may involve exposure to human blood, body fluids or tissues. Job duties may include performing or assisting in emergency medical care or first aid.
