Location: Oahu, Hawaii (must have reliable transportation — we serve the entire island)

Responsibilities

Provide skilled nursing visits in patients’ homes Complete assessments, care plans, and patient education Perform wound care, medication management, and coordination of care Communicate with physicians and the interdisciplinary team Document accurately and on time in a Medicare home health environment

 

Requirements

Current Hawaii RN license 1+ year clinical experience (home health preferred) Strong assessment and documentation skills Valid driver’s license + reliable transportation Ability to work independently in the field
