Territory - Lincoln County, St Charles County, and occasionally St Louis County (Brentwood Office); Up to a 50 mile radius
$2,500 Sign On Bonus | Part-Time | No Weekends + No Holidays

Conduct health assessments to identify medical issues and determine appropriate plans of careReview, confirm, and update plans of care to ensure services meet consumer needs and regulatory requirementsProvide supervision, orientation, and ongoing clinical oversight to homemakers, personal care aides, and home health aidesCoordinate with Client Care Coordinators, clinical staff, and external agencies to ensure quality and continuity of carePerform start-of-care and routine supervisory visits in accordance with state and federal guidelinesMonitor and report significant changes in consumers’ physical or mental statusEducate caregivers and consumers using sound nursing principles and teaching strategiesEnsure compliance with HIPAA, licensure, payer requirements, and agency policiesMaintain accurate, timely clinical documentation and audit readinessParticipate in staff meetings, in-services, and continuing educationSupport caregiver compliance, performance assessments, and quality improvement initiativesRepresent Elara Caring professionally while fostering positive relationships with clients, caregivers, and partnersPerform additional duties and projects as assignedWhat is required?
Graduate of an accredited School of Nursing (required)Minimum of one (1) year of nursing experienceHome Health, Hospice or Home care experience preferredActive RN license in good standingCurrent CPR certificationStrong clinical assessment and documentation skillsExcellent communication, organizational, and time-management skillsAbility to multitask effectively in a fast-paced environmentAdvanced proficiency in Microsoft Office (Word, Excel)Professional verbal and written communication skillsTeam-oriented, adaptable, and self-directedAbility to motivate and educate caregiversReliable transportation with the ability to travel occasionally to support audits or operational needsAbility to sit, stand, bend, lift, and move intermittentlyAbility to lift up to 50–100 lbs as required#ElaraGA
