Registered Nurse (RN) - Community Health Care A Van - St. Elizabeth Health Center

Our hospitals, care sites and clinicians are recognized for clinical and operational excellence. **RN** **Community Health** **Job Summary:** Work is performed under the general direction of the Director of Community Health for Youngstown Market and the medical direction of the Medical Director of the Care A Van or other attending physician. This is a position providing oversight and management of the Care-A-Van nursing, paraprofessional staff working on the mobile medical unit. RN will monitor nursing students and/or volunteers. RN will be responsible in providing medical care in scope of practice. RN will be responsible for stocking and ordering needed supplies.Under supervision and according to established nursing procedures, the RN Community Health works to improve the health of individuals and populations by administering screenings and vaccines, focusing on health promotion, disease prevention, and supporting health education. They provide services in a variety of settings to populations which may include pediatric, adult, and geriatric age groups. **Shift:** **Part time days** **24 hours (8 hours days)** **Essential Functions:** + Responsible for planning, organizing, directing and evaluating comprehensive nursing services within the Care A Vans programs. + Supervises and coordinates activities of nursing and van oversite. + Serves as liaison between staff and leadership personnel with emphasis on maintaining optimum communication. + Develops or revises clinical guidelines and procedures for nursing staff. + Supervises nursing and paraprofessional staff with responsibilities to include interviews, hires, and orients new employees; teaches, provides guidance, conducts performance evaluation, and terminates nursing staff. Assigns and direct daily work responsibilities for Care-A-Van staff. + Assists Medical Director with planning, coordinating, and implementing a wide variety of programs within the scope of Community Health Services. + Interacts with Mercy Health personnel; local and state community health and social service agency representatives; local school personnel as required. + Ensures the Care-A-Van program complies with applicable standards of care, legal guidelines and safety guidelines. + Collaborates with Quality Assurance/IT nurse to monitor quality of care to patients. + Periodically reviews nursing/pct documentation in patient electronic medical record. + May provide direct care to patients, in supportive role to nursing staff, including independent judgment in assessing patient care situations and assuring appropriate care. May serve as charge nurse at some clinic sites, depending on staffing needs. + Completes monthly check-ins, mid-year and annual performance reviews for direct reports. Develop results-oriented goals. + Attends and participates in meetings, workshops, seminars and external professional organizations to keep abreast of existing and new developments, procedures and techniques. + Arranges for educational programs for nurses, related professional workers, and community groups to meet needs of personnel and practitioners. + Maintains knowledge of current public health/community health issues and trends. + May assists director in preparation of program budget. + May plan for and participate in field research related to community health nursing. + Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. + Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures. + Provides patient care to pediatrics, adolescents, adults, geriatrics + Delivers services and health education in community-based settings, including homes, schools, clinics, shelters, and outreach events. + Travels to various locations to administer on-site health screening services. + Interprets results of on-site health screening services, assesses participants’ needs, and educates on/suggests lifestyle changes based on assessments. + Assists with the set up and tear down of screening site as needed. + Identifies emergency situations at the screening site and takes appropriate action. + Educates on topics such as disease prevention, chronic disease management, maternal and child health, nutrition, hygiene, medication adherence, lifestyle changes, and other health topics. + Connects participants with appropriate healthcare services and community resources for follow-up care and support as needed. + May have occasional weekends depending on event _This document is not an exhaustive list of all responsibilities, skills, duties, requirements, or working conditions associated with the job. Employees may be required to perform other job-related duties as required by their supervisor, subject to reasonable accommodation._ **Education:** + Current licensure in the State of Ohio as a Registered Nurse is required: plus, clinical and supervisory nursing experience (public health nursing a plus). **Required Licensing & Certifications:** + Current RN license in state of Ohio they are working or covered by compact (required) + BLS Basic Life Support (preferred upon hire, must obtain from approved source prior to direct patient care; ACLS Advanced Cardiac Life Support acceptable in lieu of BLS) + Clinical and supervisory nursing experience (public health nursing preferred). + Knowledge of the principles, practices, and techniques of professional public health nursing. + Considerable knowledge of physical assessment, pathophysiology, preventive health, and health education as well as acute and chronic health conditions. + Knowledge of the principles of personnel management with ability to train, supervise & motivate staff in a manner conducive to efficient & effective work performance & positive morale. + Ability to deal effectively with individuals representing a broad socioeconomic spectrum. + Education and experience in working with culturally diverse, underserved populations (preferred). + Ability to prepare reports and to communicate effectively both orally and in writing. + Possession of a valid Ohio driver's license with good driving record, and dependable personal vehicle to conduct Care-A-Van business. + Maintains telephone at residence. + Must have proven leadership skills, ability to delegate responsibility and to acquire a comprehensive understanding of all policies, procedures, systems and equipment used on the unit. + Ability to gather and analyze data. Understand basic statistics.
