What You’ll Do

As a Registered Nurse Home Health, you will:

Deliver skilled nursing care aligned with Elara Caring’s mission and clinical standards

Manage a caseload by assessing, planning, coordinating, implementing, and evaluating patient care

Complete initial and ongoing patient assessments, identifying physical, psychosocial, environmental, and spiritual needs

Develop and revise individualized plans of care based on patient needs and progress

Communicate significant findings and changes in condition to physicians, Clinical Supervisors, and interdisciplinary team members

Conduct follow-up visits based on patient, family, and facility needs

Ensure patient and staff privacy and confidentiality in compliance with HIPAA

Collaborate closely with caregivers, families, and the interdisciplinary care team

What is Required?

Current, unrestricted RN license in the state

At least one year of nursing experience in a clinical care setting

Ability and willingness to travel within the branch coverage area (up to 50%)

Dependable vehicle, valid driver’s license, and current auto insurance

Associate or Bachelor’s Degree in Nursing preferred

Home Health or Hospice experience highly preferred

Experience with HomeCare HomeBase preferred

Ability to safely push, pull, lift, and move up to 50 lbs to 100 lbs
