Job Overview:
The SCC ISNP program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in a facility setting. The Optum RN within SCC provides care to our highest-risk health plan members and is part of an interdisciplinary team. Optum is transforming care delivery with innovative and personalized care. Primary Responsibilities:
Ensure accurate and complete patient assessmentsCommunicate and collaborate with the interdisciplinary care teamAssist the provider / team with various care coordination activitiesAssist the provider / team with benefit determination associated with Medicare part A, part B, and part D benefitsResponsible for collaborating with the primary advanced practice clinician (APC) and facility to identify and respond to patient change in conditionAssist the provider / team with therapy coordinationParticipate in facility-based meetingsSchedule and participate in family conferences, team meetings, and team case presentationsCoordination of training, tracking, and compliance with quality measuresAssist in creating and conduct in-services for selected audiencesAssist providers in assuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentationMay write verbal orders from Provider in the chart in compliance with the state RN practice laws, and in compliance with individual nursing facility practicesAssist in managing members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventionsAssist in managing members during a short term sub-acute rehabilitation episode

Required Qualifications:
Associate degree or higher in Nursing (RN)Active and unrestricted license in the state which you reside, or ability to obtain by start dateCertified in Basic Life SupportDemonstrated ability to complete physical requirements, including lifting a 30-pound bag and ability to perform physical assessmentDemonstrated ability to travel in between facilitiesAccess to reliable transportation
Preferred Qualifications:
1+ years of clinical experience in practice (acute care setting preferred)Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needsSaline Lock insertion experience (preferred)Understanding of Geriatrics and Chronic IllnessProficient computer skills including the ability to document medical information with written and electronic medical recordsDemonstrated ability to work with diverse care teams in a variety of settingsProven effective Communication skills and time management

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will range from $58,800 to $105,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
