This position is responsible for the management of the Resident Assessment Instrument (RAI) Minimum Data Set (MDS) process including but not limited to ensuring the accurate, timely completion and electronic submission of the required data; auditing documentation to ensure accuracy of the data submitted; coordination of the Interdisciplinary Team for development and implementation of care plans based on data collected (Care Area Assessments) and submitted; assessment of residents' conditions to ensure proper documentation and Medicare/Medical Assistance benefit utilization.  The RNAC is also responsible for tracking and reporting information on all facility residents for performance improvement and fiscal accountability.  Duties will be performed according to the highest ethical standards and in adherence to the Corporate/Facility Compliance Plan.

 


Qualifications

Must be a graduate of an accredited Licensed Practical Nursing program OR Possess a Registered Nurse (RN) degree/license from an accredited college, university, associate’s degree program, or diploma from an accredited school of nursing.  Required to maintain an active, current RN nursing license or LPN license in the Commonwealth of Pennsylvania.  Must possess a current, valid license for the state of Pennsylvania, and have practiced nursing for a minimum of two years.  Must have an understanding of state and federal regulations governing skilled nursing facilities, Quality Indicator/Measures reports, and quality improvement initiatives.Working knowledge of computers, Microsoft Word and Excel required.
