This RN position is essential in managing acute and chronic patient care for post-transplant patients. Provide skilled observations, telephone triage and health assessments of patients including history, appropriate interventions, and referrals.Foster continuity of care and facilitate transitions across outpatient and community providers on patient population.Potential to support outreach efforts.

Knowledge and skills providing patient and family education and reinforcement related to transplant processes, including but not limited to telephone intervention and face-to-face meetings in both individual and group settings.Proficiency with typing and electronic health record documentation. Ability to manage complex clinical issues utilizing assessment skills, protocols, and evidence-based interventions. Actual Nursing Framework LEVEL and salary will be determined at time of hire.
Nursing Framework levels range from Level A to Level F.
Required qualifications must be met by the candidate in order to be interviewed and considered for the position.

Required Qualifications*Current licensure to practice nursing in the State of Michigan.MichiganThree years of nursing experience as an RN within the last 5 years.Care of immunocompromised patients within the last 5 years.Experience in caring for transplant patients as a RN within the last 2 years.Transplant Certification (CCTC/CCTN) within three years of hire.Desired Qualifications*Experience with using the United Network Organ Sharing system. 2-5 years' experience in the ambulatory care setting. 1-3 years?
