Position will be split between Wheaton and Park Ridge

Days would be just M-F and it will most likely be 2 days in Wheaton and 2 days in Park Ridge, with a flex day

Pay Range

$28.05 - $42.10

Major Responsibilities: Responsible for providing diabetes self-management education and medical nutrition therapy to AMG patients and their significant others. 1)Conduct and document individualized assessment of pertinent medical, psychosocial, and educational history of patients. 2)Educate patients and their families and / or significant others in diabetes and nutrition education needed per visit type. 3)Implement and document teaching plan, educational interventions, and follow-up plan based on the individualized assessment for patients and their families and / or significant others per current program educational curriculum or current evidence based recommendations. 4)Collaborate with patients and their families and / or significant others in establishing mutual learning goals and behavior change goals. 5)Document evaluation of outcomes and revises interventions and follow-up plan for patients and their families and / or significant others. 6)Assure education is based on current program curriculum, ADA guidelines for a nutrition care plan, and input from physicians. 7)Communicate education and follow-up recommendations to physicians and other health care providers within the multidisciplinary team. 8)Bill third party payers for diabetes outpatient self-management and training and medical nutrition therapy, utilizing appropriate coding guidelines Serve as a resource in diabetes management and nutrition education for the healthcare team. 3)Participate in formal or informal educational programs regarding diabetes and nutrition related topics. 4)Maintain knowledge of current developments, trends and techniques in nutrition therapy and diabetes management and education and makes recommendations regarding use in diabetes program. 5)Develops educational materials and displays. Obtains current literature from designated vendor for patient use on site or to take home for reference. 1)Serve as a resource person and mentor to staff related to clinical knowledge of diabetes management and nutrition therapy. 2)Act as a resource to other departments and disciplines within AMG and community. Growth and Development. 1)Keeps abreast of current standards of care via literature, educational events and internal and external resources 2)Assists in annual Core Competency by self assessment of performance and identifying future learning needs 3)Maintains active licensure and registration status (and CDE if applicable) 4)Collaborates with physicians in planning or implementing research findings into nursing protocols. 5)Participate in professional organizations related to the subspecialty in order to promote and maintain current standards of care and patient education. Licensure: Dietitian Nutritionist Education/Experience Required: - Bachelors in Dietetics, - 1-3 years experience in clinical specialty - Able to respond to the age appropriate psychological and physical needs of patients. - Understands growth and development needs of the age group with which the associate interacts. Knowledge, Skills & Abilities Required: - Ability to communicate and work cohesively with diverse groups. - Excellent organizational and planning skills. - Current licensure in the State of Illinois as a registered dietitian. - previous diabetes education experience Physical Requirements and Working Conditions: - Ability to travel locally. - Ability to function in a fast paced environment with established time constraints. - Ability to handle multiple tasks and complete work within a short time frame. - Resilient and flexible in a changing environment. - Ability to cooperate and work well with others. - Ability to make sound and timely decisions. - Ability to flex hours in order to accommodate needs of patients (to include evening and weekend hours) If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
