Position Highlights As a Social Services Specialist Rover, you serve as a champion for resident rights and mental well-being. Your core mission is to provide a seamless psychosocial experience through the following: Clinical Advocacy: Conducting comprehensive assessments and developing interdisciplinary care plans that honor the strengths and needs of every resident. Navigational Support: Acting as the lead architect for discharge planning by connecting residents with community resources and ensuring a safe, confident transition home. Crisis & Transition Management: Providing therapeutic interventions to help residents and families navigate the emotional journey of long-term care. Regulatory Excellence: Ensuring all documentation (MDS, Social Histories, and Care Plans) is precise, timely, and compliant with NASW and government standards. Collaboration: Working in collaboration with nursing, therapy, and administration to provide a holistic approach to health. Qualifications *Bachelors degree from accredited school of Social Work *Must possess any certifications/licensures as required by State of employment to practice in long term care. *One to three years of supervised social work experience in health care setting working directly with individuals preferred. *Additional certification such as Geriatric Case Management, Hospice & Palliative Care, Gerontology, Clinical Social Work, Health Care, Nephrology, Mental Health, and/or Substance Abuse preferred.
