Overview

The Quality Nurse Care Mgr supports population health and value-based care initiatives by identifying, tracking, and validating closure of quality gaps across assigned patient populations. This role works closely with Care Management and Value-Based Operations teams to ensure preventive and chronic care measures are addressed and accurately documented.

Responsibilities Identify and track open quality gaps using registries, chase lists, and population health toolsPrioritize gaps based on program and contract requirementsValidate closure of quality measures through chart reviewSupport coordination of outreach and follow-up activitiesReview clinical records to confirm completion of screenings, labs, and preventive servicesVerify documentation accuracy and completenessMaintain quality trackers and dashboardsCollaborate with Care Management and Value-Based Operations teamsParticipate in quality huddles and performance reviewsSupport audit preparation and quality reporting activitiesProvide feedback on documentation requirementsAssist with onboarding and training related to quality workflowsAttend care huddles and review all open gaps Qualifications Minimum 2–4 years of relevant healthcare experience (population health, care management, quality improvement, or value-based care preferred)Experience with chart review and documentation validationExperience with ACOs, MA plans, or risk-based modelsFamiliarity with EMRs, registries, and quality reporting toolsActive, unrestricted compact LPN or RN licenseStrong organizational and attention-to-detail skillsAbility to follow standardized protocolsEffective communication skillsCollaborative and team-orientedComfortable working with metrics and dashboardsTravel up to 25% About our Line of Business Abode Care Partners, an affiliate of BrightSpring Health Services, is a leading provider of integrated medical services, caring for individuals from post-hospitalization to home in various settings ranging from skilled nursing facilities, assisted living, independent living, group homes, and private homes.
