Note: The compensation range(s) in the table below represent the base salaries for all positions at a given grade across the health system. Typically, a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries may vary by position and will be determined based on the candidate's relevant experience. No employee will be paid below the minimum of the range. Pay ranges are listed as hourly for non-exempt employees and based on assumed full time commitment for exempt employees. Minimum - Midpoint - Maximum $28.73 - $33.02 - $39.05 **Summary** Under the direction of the Director of Population Health and Clinical Integration, the Program Coordinator of the HNE BeHealthy ACO will be a key member of the Care Management ACO leadership team, supporting the operations and continued growth of the Medicaid community-based accountable care model. This position is responsible for the programmatic success of all BeHealthy ACO CM teams across the Baystate Health enterprise. The incumbent works collaboratively with both the clinical managers of the BeHealthy CM ACO team, as well as other clinical departments to sustain and grow a unified program that links services across all departments for successful management of a designated population. The position is responsible for assisting to develop and maintain an operational business plan and strategy that grows the clinical program, and contributes to the financial, quality, patient experience, and overall success of the program. The Program Coordinator is responsible for the implementation of methods and workflows to operationalize and improve overall effectiveness and efficiency of the CM teams. In addition, the Program Coordinator is responsible for ongoing team productivity through regular data analytics collection and oversight, sharing and educating both peers and team members regarding trends, opportunities and highlighting best practices. The Program Coordinator will provide supervision, coordination, technical assistance, consultation and other resources on the development and operations of all CM programs based on the assessment of community needs for HNE BeHealthy members, as well as their expected health outcomes. The successful incumbent will provide ongoing communication and education to all ACO BeHealthy CM teams related to operational success, and collaborate alongside the clinical team managers, creating a strong and unified leadership team. Adheres to system and department compliance policies, and any and all applicable laws and regulations. Performs other duties as assigned. **Requirements** 1. Contribute and assist CM leadership with strategic program growth and development: Demonstrates understanding of clinical program elements, payment and cost structure, clinical volumes, market forces and operational structure for program success. Integrates, aligns, and synthesizes program plans across traditional departmental lines, in accordance with both NCQA and MassHealth contractual requirements. 2. Contribute and assist CM leadership with HNE BeHealthy ACO Care Management Quality Program: Regular oversight and management of the NCQA and MassHealth quality program requirements and quality measurements. Assist as a key contributor to ongoing participation of staff in applying the principle/model of continuous improvement to improve existing processes and enhance the effective utilization of resources. 3. Contribute and assist CM leadership with Contract knowledge / Accrediting body content expertise: Keen awareness of contractual agreement expectations for both NCQA and MassHealth auditing entities, ensuring compliance with all quality and operational details included within each regulatory body. Presents self as a resource to all team members for contractual related workflows and operational practice functions. 4. Operations and Programmatic Work: Monitor effectiveness of current workflows and productivity related to each BeHealthy ACO CM team. Responsible for day to day operational functions of the CM program related to volume of care managed patients, oversight of care management community partners and services available to care managed patients, and responsible for establishing and maintaining strong collaborations with current and future external partners. 5. Data Analytics: Demonstrates knowledge and functional ability in collection and oversight of weekly/ monthly productivity data for all teams. Creation and maintenance of a CM dashboard, including key metrics demonstrating operational effectiveness, opportunities for improvement, and best practice of maximizing resources and skills of the teams. Provide education to both leaders and team members regarding interpretation of data, including next best steps related to data outcomes. 6. Health New England Partnership: Functions as key participant and liaison in fostering ongoing collaborations with all HNE partners related to the BeHealthy ACO CM program. Point of contact for all related care management operational functions, communications (i.e. announcements, newsletters), education needs, and all other ACO CM needs. Serves as a main resource between HNE and CM leadership. 7. Care Management Education: Functions as key participant with both ongoing educating and orienting new team members upon hire. Ensures new team members are well equipped with an orientation plan encompassing all aspects of their role, and are set up for success with the tools and resources needed. Works collaboratively with clinical CM managers in providing the needed onboarding, education, shadowing opportunities, and any/all other resources to create well rounded and knowledgeable team members. 8. Administrative Support: Provides administrative support and collaboration to ACO CM leadership as needed. Meets on a regular basis with CM leadesrhip to coordinate and assist in planning departmental meetings and agendas related to the success of the CM team. In conjunction with CM leaders, acts as a key communicator to all CM teams, ensuring communications are well shared and understood by teams. Functions as the key CM spokesperson as needed to all internal/external partners to ensure program success. **Skills/Compitencies** 1. Excellent oral, written, and presentation communication skills.
