Position Summary
The Program Coordinator (QIDP) is responsible for the overall day-to-day operation of the Day Habilitation Program. This role provides leadership, oversight, and decision-making to ensure services align with the agency’s mission, comply with all regulatory requirements, and operate within approved budgets. The Program Coordinator reports to the Program/Site Director of Day Habilitation.

Essential Responsibilities

Provide supervision and leadership to Supervising Habilitation Specialists, Habilitation Specialists, Assistants, and per diem staff.

Participate in recruitment, interviewing, and hiring recommendations for program staff.

Ensure staff are trained in customer satisfaction, program operations, and regulatory compliance, including OPWDD, OMIG, and agency policies.

Oversee development and implementation of program plans and curriculum.

Ensure required documentation is completed, reviewed, and available for audits, including CFAs, Life Plans, reviews, and Level of Care determinations.

Conduct QIDP reviews to ensure timely submission of billing, program plans, monthly summaries, and survey documentation.

Complete performance evaluations, manage corrective action processes, and maintain required personnel records.

Utilize electronic health record systems and management tools to support service delivery, documentation, referrals, and confidentiality compliance.

Serve as a point of contact for crisis intervention for staff and community providers.

Collaborate with referral sources, care providers, and transportation agencies.

Represent the agency professionally at all times.

Additional Responsibilities

Develop and oversee operational systems, including emergency and evacuation procedures.

Conduct regular staff meetings and ensure accurate weekly census reporting.

Oversee systems for screening, admissions, discharges, referrals, ISP reviews, and staffing.

Monitor participant progress through observation and staff oversight.

Participate in assigned committees.

Provide physical support as needed, including SCIP interventions, CPR/First Aid, and emergency procedures.

Maintain confidentiality and perform other duties as assigned.

Bachelors Degree required and at least two years supervisory experience preferred. Masters degree in Special Education, Rehabilitation Counseling, Social Work or related field preferred, Demonstrate proficiencies in Electronic Health Records and other: Valid New York State Drivers license for at least 2 years required and access to a vehicle. Excellent communication skills and customer service skills.
