Shift Information:   PRN Day Shift Monday through Friday 8 AM – 5 PM - (1 to 2 days per week eight hour days)Home Office Location: - Mason Ridge in ChesterfieldCoverage area: Supporting All BJC Hospitals in the St Louis areaCase Load: Varies but there is support in covering caseloads with other Social Work team membersJob Responsibilities: Completing SW assessments and discharge planning for GIP patients admitted to the hospital. Assisting with discharges home on routine hospice. Placement needs. Assistance with financial applications. Helping patients and families understand insurance benefits. Consents. Supporting families with anything related to Hospice care. Responding to new consults and being able to give information on BJC HospicePreferred skills or required work experience:  Previous Hospice or Hospital/Inpatient SW experience preferred
Overview

BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.

Hospice, the final stage of BJC's continuum of care, is a special kind of caring for patients with a life-limiting illness. Services are provided in the comfort of the patient's home or skilled nursing facility. The Hospice staff are sensitive to the physical, psychosocial, emotional and spiritual needs of terminally ill adult and pediatric patients and their families. We provide a multi-disciplinary team of healthcare professionals and volunteers, specially trained in symptom management, pain control, counseling and bereavement services for the dying. Our Hospice services include alternative therapies such as music, art and massage therapy. Our Hospice programs provide palliative care by helping patients manage their pain and symptoms while living their lives with daily peace and dignity.

Role Purpose

Provides clinical social work services to address the psychosocial needs of patients and families. Provides individual and group counseling, linkage to community resources, expertise in cultural diversity and advocacy. Collaborates with the health care team and ensures timely, cost-effective discharge and transition planning.

 

Responsibilities

Conducts screening and assessment activities to meet the needs of the discharged patient.Applies assessment findings to prepare a patient-centered plan of care.Applies and complies with all regulations and utilizes resources available to patients to manage their continuum of care.
