Position Summary:
We have an exciting opportunity to join our team as a Post Acute Care Associate - Care Management - Mineola.

In this role, the successful candidate coordinates and facilitates patients discharged to home with transportation, durable medical equipment delivery arrangement, and managing long-term care plans.

Job Responsibilities:

Performs intake of referrals for post acute care services from case management and social services departments' staff.Completes intake form for patients with managed long-term care (MLTCP) to obtain post acute care services and providers.Communicates with interdisciplinary team and patient designee regarding status of post acute care plan implementation.Maintains and documents all activities in the patient medical record ( Allscripts) in accordance with the institution s established policies and standards.Facilitates gathering and dissemination of clinical information and required completed forms to payers and post acute care providers in collaboration with the interdisciplinary team and case manager overseeing the patients discharge plan. Maintains certification to complete New York State Department Of Health SCREEN forms as necessary for patients discharged to skilled nursing facilities.Communicates with payors to determine the status of approval or denial of requested post acute care services. Follows up with the actual post acute care service provider to provide outcome.Obtains authorizations from payors and coordinates transportation services for patients as needed. Obtains authorization from payers and coordinates the ordering and delivery of durable medical equipment (DME).

Minimum Qualifications:
To qualify you must have a Bachelor's Degree Social Science, Liberal Arts, Healthcare or related field. 1- 2 years Hospital and/or Healthcare setting or Related Human Services. Strong communication skills (both oral and written) detail orientation, organization, planning, analytical, customer service, Problem-Solving skills, and multitasking. Strong computer skills; knowledge of Microsoft Word and Excel.

Preferred Qualifications:
Department of Health SCREEN.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $48,682.22 - $81,137.04 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
