About the Role
We’re looking for a tech-savvy Population Health Coordinator to work remotely while living in the Orlando, FL area. You’ll support patient outreach and care coordination to help improve health outcomes.
Schedule
Monday–Friday, 8:00 a.m.–4:30 p.m.
Ideal Background & Skills
Bilingual in Spanish or Creole strongly preferred
Medical Assistant or front-desk/health-clinic experience
Proficient with Excel and PowerPoint; comfortable with technology and data
Strong communication skills and understanding of clinical terminology
What You'll DoUtilizes verbal and written communication skills to develop rapport and trust with membersPerforms member outreach via telephone, secure messaging, and/or other approved communication methodsMonitors internal work queue and other reports to contact and schedule members where appropriateDocuments outreach touchpoints and relevant notes into the electronic medical recordConsults with other members of the healthcare team to solve problems and seek adviceMaintains member privacy and confidential member informationMay require other duties as assignedAdditional skills that may be required:Collaborates with client, digital wellness center and care managers to coordinate and promote health and wellness programsComfortable using ExcelWhat You'll BringHigh School Diploma or GED required; Associate’s Degree or equivalent work experience preferredCompletion of a Medical Assistant training program preferredWork-life balance is at the foundation of how decisions are made and where Premise is headed.
