What You’ll DoPromote Elara Caring’s philosophy, mission, and policies to ensure high-quality patient care
Provide direct patient care, including evaluations, treatment planning, and implementation
Assess patients’ functional needs and adjust care plans to optimize outcomes
Design and implement individualized Plans of Care with patient and caregiver involvement
Practice therapy within defined standards of care and state regulations
Direct and supervise support staff to help patients achieve functional goals
Complete accurate and timely documentation, including evaluations, treatment goals, and updates
Serve as a clinical resource and advocate for physical therapy within the community
Physical & Work RequirementsAbility to sit, stand, bend, lift, and move intermittently
Ability to lift and transfer 50–100 pounds
Physically demanding, fast-paced work environment
May include irregular hours or call coverage, as applicable
Willingness to travel within the branch coverage area (up to 50%)
What Is Required?Graduate of an APTA-accredited Physical Therapy program
Current, unrestricted PT license in the state of practice
Minimum of one (1) year experience in a healthcare setting
Reliable transportation, valid driver’s license, and current auto insurance
Ability and willingness to travel within assigned territory
You will report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
