Your Role as a Physical TherapistIn this role, you will contribute to our mission by delivering high-quality physical therapy services and supporting optimal patient outcomes. Key responsibilities include:
Promoting Elara Caring’s philosophy, mission, and administrative policies to ensure consistent, high-quality care
Providing direct patient care, including functional evaluations, treatment planning, and outcome assessment
Collaborating with interdisciplinary care teams and consulting with other specialists as needed
Designing and implementing individualized plans of care based on comprehensive patient assessments
Adjusting care plans as patient needs evolve to maximize functional independence
Directing and supervising support staff to help patients achieve functional goals
Maintaining accurate, timely documentation of evaluations, treatment plans, progress notes, and required signatures
Acting as a clinical resource and providing leadership to promote physical therapy services within the community
Work Environment & Physical RequirementsPhysically demanding and fast-paced clinical environment
Full range of body motion required, including lifting, transferring, and assisting patients
Ability to work irregular hours, including on-call responsibilities if applicable
Ability to sit, stand, bend, lift, and move intermittently
Must be able to lift 50–100 lbs
Qualifications & RequirementsGraduate of an APTA-approved Physical Therapy program
Current, unrestricted Physical Therapist license in the state of practice
Minimum of one (1) year of experience in a healthcare setting
Willingness and ability to travel within the assigned branch or coverage area (up to 50% travel)
Valid driver’s license, reliable transportation, and current auto insurance in accordance with state laws
Reporting StructureThis position reports to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
