Your Role as a Physical Therapist

In this role, you will contribute to our mission by delivering high-quality physical therapy services and supporting optimal patient outcomes. Key responsibilities include:

Promoting Elara Caring’s philosophy, mission, and administrative policies to ensure consistent, high-quality careProviding direct patient care, including functional evaluations, treatment planning, and outcome assessmentCollaborating with interdisciplinary care teams and consulting with other specialists as neededDesigning and implementing individualized plans of care based on comprehensive patient assessmentsAdjusting care plans as patient needs evolve to maximize functional independenceDirecting and supervising support staff to help patients achieve functional goalsMaintaining accurate, timely documentation of evaluations, treatment plans, progress notes, and required signaturesActing as a clinical resource and providing leadership to promote physical therapy services within the community

Work Environment & Physical Requirements

Physically demanding and fast-paced clinical environmentFull range of body motion required, including lifting, transferring, and assisting patientsAbility to work irregular hours, including on-call responsibilities if applicableAbility to sit, stand, bend, lift, and move intermittentlyMust be able to lift 50–100 lbs

Qualifications & Requirements

Graduate of an APTA-approved Physical Therapy programCurrent, unrestricted Physical Therapist license in the state of practiceMinimum of one (1) year of experience in a healthcare settingWillingness and ability to travel within the assigned branch or coverage area (up to 50% travel)Valid driver’s license, reliable transportation, and current auto insurance in accordance with state laws

Reporting Structure

This position reports to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
