The Ambulatory Care Clinical Specialist Pharmacist in Inflammatory Bowel Diseases (IBD) at Cedars-Sinai Medical Center in Los Angeles, California is an Advanced Practice Pharmacist who works with a multidisciplinary team to provide comprehensive medication management for IBD patients. Responsibilities include medication reconciliation, therapy monitoring, managing side effects, educating patients on specialty medications and infusions, and assisting with patient access.
The Pharmacy Specialist in IBD achieves positive patient outcomes through evidence-based disease management guided by established protocols, promotes adherence, and enhances satisfaction among both patients and based on current clinical guidelines, such as those from the American College of Gastroenterology and Cedars-Sinai’s P&T-approved protocols, the specialist initiates, adjusts, or discontinues therapies, orders and interprets lab results, and authorizes medication refills.
Additionally, this specialist educates patients about specialty medications and serves as a clinical resource for physicians and support staff and liaison with the Cedars-Sinai Medical Center Specialty Pharmacy. They also help develop and maintain order sets, best practices, protocols, competency assessments, and participate in ongoing quality improvement efforts.
