We are seeking a dedicated and knowledgeable Registered Nurse to join our team as a Peritoneal Dialysis/Home Hemodialysis Training Nurse. This position is centered on empowering patients and their families to manage dialysis treatments independently at home. The ideal candidate will possess exceptional teaching skills, a strong commitment to patient care, and a proactive approach to preventing complications.

Key Responsibilities Patient & Care Partner Training Deliver thorough, hands-on training for peritoneal dialysis (PD) and home hemodialysis  Educate on aseptic techniques, catheter care, fluid exchanges, machine operation, and infection control. Customize teaching methods to accommodate various learning preferences, including those of family members and caregivers. Home Assessments & Follow-Up Conduct initial and yearly home visits to assess the suitability of the environment, and ensure proper supply management and technique. Provide monthly clinic reviews, telephonic support, and on-call troubleshooting services to address potential issues such as peritonitis. Case Management Monitor patient progress through lab results, vital signs, and treatment adherence. Collaborate with a multidisciplinary team, including nephrologists, dietitians, and social workers, to develop personalized care plans. Quality & Compliance Ensure adherence to regulatory standards for home dialysis as per federal and state requirements. Maintain detailed documentation in electronic medical records and report any incidents promptly. Qualifications Active RN license. RN with experience in PD training. Minimum of one-year nursing experience, with expertise in hemodialysis and peritoneal dialysis. At least six months of general dialysis experience. CDN (Certified Dialysis Nurse) preferred. BLS/ACLS required.
