This contract will provide occupational medicine health services and electronic health records management for sworn staff from both the City of Phoenix (City) Fire and Police Departments as well as public safety personnel assigned to other jurisdictions throughout the region who have contracted with Phoenix Fire Department Health and Wellness Center (PFDHWC) for these services.

Location:50 S. 12th St., Phoenix, AZ 85034.
PFDHWC Clinic hours:7:30 a.m. to 12:00 p.m. and 12:30 p.m. to 4:00 p.m. Monday through Friday, with a 30-minute lunch period.
Responsibilities:
+ Assist Providers, Nurse Case Manager and Case Coordinator with expediting appointments to appropriate specialist for injury type and severity.
+ Facilitate rehabilitation as needed based on severity of injury and PFD member’s need.
Requirements
Minimum Qualifications:
+ Five years of experience working as a medical patient liaison.
+ Must have established relationships with physicians in all specialty areas in the valley.
+ Strong organizational skills and effective oral and written communication skills.
+ Working knowledge of most current surgical techniques.
This job is CONTINGENT upon contract award, client approval, completion of a favorable background investigation, and the ability to obtain and maintain a Public Trust / High Risk clearance.
Final compensation will be based on relevant factors including work experience, skills, certifications, and internal equity.
