Key Responsibilities: Lead and manage the delivery of patient care services, ensuring compliance with all regulatory standards. Provide supervision, mentorship, and support to nursing staff to promote professional development and optimal patient outcomes. Collaborate with interdisciplinary teams to create and implement personalized patient care plans. Ensure precise and timely documentation of patient care and progress. Conduct regular audits of patient records and care plans for quality assurance. Serve as a communication link between patients, families, and healthcare providers to ensure seamless care coordination. Engage in continuous quality improvement initiatives and professional development activities. Required Qualifications: Previous experience in home care setting with two years management or supervisory experience, preferred.
