Patient Care Coordinator, Specialty - Evernorth

**On‑Site Training:**
**_This role requires 6-8 weeks of on‑site training at the following location:_**
**_1001 Oakdale Road, Oakdale, PA 15071_**
Responsible for supporting the clinical management and refill coordination of specialty pharmacy patients under the supervision of a licensed pharmacist. This role involves direct communication with patients, caregivers, and healthcare professionals to ensure accurate and timely delivery of medications.
**Responsibilities**
+ Coordinate patient deliveries by making outbound and receiving inbound calls from patients, caregivers, and medical staff.
+ Conduct follow-up calls as directed by a licensed pharmacist.
+ Assist the specialty pharmacy team with tasks such as:
+ Evaluating product usage
+ Managing and documenting supply inventory
+ Setting up medication orders
+ Performing verbal patient assessments
+ Maintain and update patient records, medication profiles, and complete assessments under pharmacist supervision.
+ Accurately document all interactions in patient medical records.
+ Meet or exceed quality and productivity standards (e.g., 100–125 form setups daily).
+ Deliver exceptional customer service and maintain a professional, service-oriented approach in all interactions.
+ Act as a liaison between specialty pharmacy, operations, and patients, caregivers, or medical professionals.
+ Adhere to system-wide competencies and behaviors.
+ Perform additional duties as assigned.
**Requirements**
+ High school diploma or GED required.
+ Pharmacy Technician Certification preferred.
+ Prior experience in specialty healthcare preferred.
+ Proficiency in Microsoft Office Suite.
As part of Evernorth Health Services, this role supports CarepathRx within
our Pharmacy and Care Delivery organization. Our team focuses on
specialty pharmacy and infusion services in partnership with hospitals and
health systems.
If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.
For this position, we anticipate offering an hourly rate of 20 - 30 USD / hourly, depending on relevant factors, including experience and geographic location.
This role is also anticipated to be eligible to participate in an annual bonus plan.
