Overview

Northwest HealthCare, six miles west of Christian, offers 24- hour emergency care, the Sleep Disorders Center and a variety of outpatient diagnostic and imaging services.

 

The Emergency Medical Services Department is charged with the delivery, management and transport of patients involved in traumatic and medical emergencies.


Preferred Qualifications

Role Purpose

A Mobile Integrated Health (MIH) paramedic has a comprehensive medical knowledge base and is able to provide post hospitalization care for a broad range of medical conditions. The MIH paramedic will also be able to provide advanced level medical care including; post discharge evaluations, primary care in the home, and Social Determinates of Health. Must have Illinois paramedic license and IDPH MIH program completion.

 

Responsibilities

Conducts post-hospital release follow-up care including, but not limited to, monitoring medication, dressing changes, and checking vital signs.Conducts physical exams in patient's homes and works with physicians to provide continuity of care.Observes, records, and reports to physician, patient’s conditions and reactions to drugs, treatments, and significant incidents.Works autonomously and in the best interest of the patient.Conducts follow-up visits to post-discharged patients to provide necessary connection to community resources and primary care while monitoring medical needs.
