Be the Best Palliative Care Nurse Practitioner You Can Be  

If you meet these qualifications, we would love to meet you:  

Graduated from an approved school of professional nursingOne year experience as a Nurse Practitioner is preferredLicensed to practice as a Nurse Practitioner in the state of agency operation Active DEA license in the state where care is being provided.Possess and maintain valid CPR certification while employed in a clinical role, preferredMust be a licensed driver who can travel to all business locationsMeet the regulations and requirements of the state(s) in which program provides services 

Responsibilities:   

As a Palliative Care Nurse Practitioner, you will: 

Develop and implement clinical plans of care for patients with chronic and terminal illnessesPerform patient assessment, history and physical; recommendations on aggressive symptom-managementMonitor patient progress; update the Medical Director, Team Director and team members; consult regarding discharge plannning; and patient/family teachingMaintain communication with the supervisory physician (s) and attending physician to review patient records and care plans on a routine basisInitiate communication with attending physicians, other Palliative Care staff members and other agencies as needed to coordinate optimal care and use of resources for patient/familyMay provide support at time of deathPrescribe medication as permitted by the state and/or the State Board of NursingParticipate in continum of care for patients that are hospice appropriate and ongoing education programs.
