POSITION SUMMARY
The PACE Life Physical Therapist evaluates participants, administers physical therapy treatments, records progress and instructs participations and/or family in care as part of the PACE Organization. This position may also directly lead the work activity of aides, assistants and students. This position works in the PACE Center and in participant's homes.
The PACE Life Physical Therapist conducts initial assessments and periodic reassessments, develops plan of care, completes coordination of medical, functional, and psychosocial condition of each participant, remaining alert to pertinent input from other team members, participants' caregivers, as well as documenting changes in a participant's medical record consistent with documentation policies.
The Physical Therapist delivers care in a prompt and professional manner always maintaining the highest level of confidentiality and respect. This position works as part of the care team to meet the needs of the customer and to support departmental objectives.
QUALIFICATIONS
EDUCATION: Bachelor's degree in Physical Therapy from an accredited Physical Therapy program required.
LICENSE/CERTIFICATION: Licensed in the State of New Jersey. HSAED required. Valid NJ Driver's License, Registration and Insurance required.
EXPERIENCE: Minimum 2 years' experience as Physical Therapist. Minimum 1 year working with the frail or elderly patient. Prior experience preferred but not required. Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.
PERFORMANCE EXPECTATIONS
Demonstrates knowledge and clinical competency with physical therapy assessment skills, treatment implementation and proficiency with modalities, as listed on the Assessment and Evaluation Tool for this position.
WORK ENVIRONMENT
Potential for exposure to the hazards and risk of the home environment, including exposure to infectious disease, blood-borne pathogens, hazardous substances, and potential injury.
Requires standing much of the day and extensive amounts of assisting, support and lifting of patients. Requires the ability to lift greater than 50 pounds.
